The following forms must be completed and submitted via mail, fax (781-320-1067), or in person to the Town Hall Human Resources Office, 580 High Street, 2nd Floor.

- Online Payroll Info / Employee Forward - For reviewing pay statements and W2 forms.
- Long Term Disability Enrollment or Waiver Form - Benefit Info

Please go to the BENEFITS PAGE of our website for enrollment forms and information on:
Health Insurance, Dental Insurance, Flexible Spending Accounts, Life Insurance, Long-Term Disability Insurance, 457 and 403(b) plans.
Please note that health insurance coverage begins on the first of the month. If your position starts on the first, it will become effective that day. If you start after the first, it will become effective on the first day of the following month.
