Expand/Contract Questions and Answers
For Town employees, you need to complete a new W4 for your federal taxes and/or M4 for your state taxes and return the completed form(s) to HR.
For School employees, log into TalentEd and complete a ‘PAYROLL_W4 Change’ Form and/or a "PAYROLL_M4 Change' Form.
Please see the last FAQ for instructions on how to return completed forms to Human Resources.
For Town employees, you need to complete a new Direct Deposit Form, and return that form, as well a voided check or bank authorization letter to HR.
For School employees, log into TalentEd and complete a ‘PAYROLL_Direct Deposit Change’ Form.
Please note that you are able to split your paycheck between two accounts, but they need to be at two DIFFERENT institutions.
For Town employees, send an email with your new address to benefits@townhall.westwood.ma.us.
For School employees, log into TalentEd and complete a ‘PERSONNEL_Address/Name Change Form.’
In Employee Forward. Upon starting with Westwood, you should have received a message in your personal email account from no-reply@employeeforward.com. If it is not in your inbox, please check your spam folder. This email will have a link and instructions to set up your Employee Forward account.
If you have any questions on or issues with your Employee Forward account, please contact Marie O’Leary (moleary@townhall.westwood.ma.us).
You may not need to make any changes, but we encourage folks who are getting married to consider if they need to make any of the following updates:
(A) Canceling health and/or dental insurance to join spouse’s plan(s):
For Health Insurance, within 60 days of your marriage, either:
- Log into the GIC member portal, select 'qualifying event' and follow the prompts from there. You will need to provide a copy of your marriage certificate as a supporting document.
- Reach out to Human Resources and we can send you a paper form to complete or one via email. You will also need to provide a marriage certificate.
For Dental Insurance, within 60 days of your marriage, either:
- Complete and return a Delta Dental Form to Human Resources, and check off ‘Termination’ at the bottom.
(B) Adding spouse to health and/or dental insurance:
For Health Insurance, within 60 days of your marriage, either:
- Log into the GIC member portal, select 'qualifying event' and follow the prompts from there. You will need to provide a copy of your marriage certificate as a supporting document.
- Reach out to Human Resources and we can send you a paper form to complete or one via email. You will also need to provide a marriage certificate.
For Dental Insurance, within 60 days of your marriage:
- Complete and return a Delta Dental Form to Human Resources, make sure to include your spouse’s information in section 2 and check off ‘New Addition’ at the bottom.
(C) Changing your tax filing status:

(D) Adding/changing the beneficiary on your Pension System:
- For Norfolk County Retirement, re-complete pages 7-10 of the Norfolk County Enrollment Form and return to HR.
- For Mass Teachers' Retirement, complete the Beneficiary Designation Form and return it directly to MTRS through the mail.
(E) Adding/changing the beneficiary on your Boston Mutual Life Insurance policy:
Complete the Certificate Change Form and return to HR.
(F) Changing your emergency contact:
- For Town employees, complete a new Emergency Contact Form and return it to HR.
- For School employees, log into TalentEd and complete a ‘PERSONNEL_Emergency Contact Change’ Form.
(G) Changing your name:
- For Town employees, send an email with your new name and a copy of your marriage certificate to benefits@townhall.westwood.ma.us.
- For School employees, log into TalentEd and complete a ‘PERSONNEL_Address/Name Change Form.’ Please note, if you are enrolled in MTRS, you must have changed your name directly with them BEFORE you complete a name change form.
You may not need to make any changes, but we encourage folks who are getting divorced to consider if they need to make any of the following updates:
(A) Removing your former spouse from your health and/or dental insurance:
For Health Insurance, within 60 days of the event, either:
- Log into the GIC member portal, select 'qualifying event' and follow the prompts. You will need to provide a copy of the divorce decree as a supporting document.
- Reach out to Human Resources and we can send you a paper form to complete or one via email. You will also need to provide the divorce decree.
For Dental Insurance, within 60 days of the event:
- Complete and return a Delta Dental Form to Human Resources, and check off ‘Remove Dependent,’ along with your former spouse’s name on the bottom.

(C) Changing the beneficiary on your Pension System:
- For Norfolk County Retirement, re-complete pages 7-10 of the Norfolk County Enrollment Form and return to HR.

Complete the Certificate Change Form and return to HR.

- For Town employees, send an email with your new name and a copy of your divorce decree to benefits@townhall.westwood.ma.us.
- For School employees, log into TalentEd and complete a ‘PERSONNEL_Address/Name Change Form.’ Please note, if you are enrolled in MTRS, you must have changed your name directly with them BEFORE you complete a name change form.
You may not need to make any changes, but we encourage folks who are having or adopting a child to consider if they need to make any of the following updates:
(A) Adding child to health and/or dental insurance:
For Health Insurance, within 60 days of the event, either:
- Log into the GIC member portal, select 'qualifying event' and follow the prompts from there. You will need to provide a copy of the birth certificate or adoption placement letter as a supporting document.
- Reach out to Human Resources and we can send you a paper form to complete or one via email. You will also need to provide the birth certificate or adoption placement letter.
For Dental Insurance, within 60 days:
- Complete and return a Delta Dental Form to Human Resources, make sure to include your child’s information in section 2 and check off ‘New Addition’ at the bottom.
(B) Adding dependent to tax forms/changing withholdings:

(C) Adding/changing the beneficiary on your Pension System:
- For Norfolk County Retirement, re-complete pages 7-10 of the Norfolk County Enrollment Form and return to HR.
- For Mass Teachers' Retirement, complete the Beneficiary Designation Form and return it directly to MTRS through the mail.
(D) Adding/changing the beneficiary on your Boston Mutual Life Insurance policy:
Complete the Certificate Change Form and return to HR.
8. I’m an active employee turning 65, and am enrolled in one of the Town’s health plans, what do I need to do?
- You should sign up for Medicare Part A (free for most people) but NOT Medicare Part B or D. You will sign up for Medicare B when you retire.
- Should your spouse turn 65 while you are still an active employee and is listed as a dependent on your health plan, they should sign up for Medicare Part A (free for most people) but NOT Medicare Part B or D. They will sign up for Medicare B when you retire.
- In case of serious injury, call 911.
- Report the injury to your supervisor. They will complete an injury report.
- For minor injuries that require medical attention, employees can go to Norwood Urgent Care at the address below. The Town will be billed directly for the care

10. I am a school employee, when do I reach out to Human Resources versus the school administration at Central Office?
Human Resources should be contacted with questions regarding:
- Benefits (health, dental, life insurance etc.)
- Taxes
- Payroll deductions
- Worker’s compensation
- Retirement planning
Central Office handles:
- Background screening
- Onboarding paperwork outside of benefits and payroll
- Time off accruals and balances
- Leaves (HR is looped in on these as well, but school admin should be contacted initially)
- Employee relations
- Payroll hours and rate
- TalentEd

11. What is considered a qualifying event that allows you to make insurance changes, and what is the process for making those changes?
- Marriage
- Birth, Adoption
- Divorce/Legal Separation
- Death of Spouse/Dependent
- Spouse/Dependent's Employer's Open Enrollment
- Spouse/Dependent Moving to the United States
- Loss of eligibility for Medicaid or CHIP coverage
- Court order/Judgement
- Moving out of your health plan's service area
- Turning 26 and losing coverage through a parent’s plan
- Change in Employment Status
Within 60 days of a qualifying event either:
- Log into the GIC member portal, select 'qualifying event' and follow the prompts from there.
- Reach out to Human Resources and we can send you a paper l form to complete or one via email.
For any qualifying event, you will need to provide supporting documentation. Please review the GIC Qualifying Chart to see which documentation/information is needed based on the event.
