To request patient information for an ambulance run you will need to make an official Freedom of Information Request to the Fire Department.

Official requests must be made through our dedicated FIOA portal. 

Requests WILL NOT be released unless you include a signed medical release by the patient or their legal guardian. Download and complete the following document HIPAA Records Release

In your request, please indicate if you will require medical billing information.

All requests for EMS ambulance & billing reports require a processing fee of $15
